Virtual Consultations
After reviewing your form, our office staff will reach out to you to schedule the online consultation. There is a $100.00 consultation fee, which can be applied to your in-office consultation. You will be emailed a secure link to connect with our team via Zoom. Please be sure to download the app prior to the scheduled appointment time.
A Zoom consultation does not replace a full evaluation in person by the physician. At the in-person visit you will have imaging and further evaluation by the physician. Patients who would like to proceed with surgery and/or further consultation will be able to set up the appointment at the time of the Zoom call. The Zoom consultation fee will be applied at the time of the follow-up in person visit.

Click here to visit our New Patient Page, and download the “New Patient Forms”. Once completed, scan and email the completed forms to [email protected], or fax to 713-526-5160.
Step 3:
We will call you and schedule a time that works for you and the physicians. Please be sure to download the Zoom (zoom.us) application prior to your appointment. We will email you a link to join the Zoom Consultation prior to your appointment.
