Radiofrequency Debulking and CO₂ Laser Resurfacing

Rhinophyma is a progressive condition that causes the skin and soft tissue of the nose to become thickened, enlarged, irregular, and sometimes bulbous. Although it is associated with rosacea, established rhinophyma is caused by structural overgrowth and thickening of the nasal skin and underlying tissue and often requires procedural treatment.

At Facial Plastic Surgery Associates, Dr. Sean Delaney and Dr. John Hamati treat rhinophyma using an individualized combination of radiofrequency tissue debulking and sculpting followed by CO₂ laser resurfacing. This approach allows excess tissue to be reduced and the nose to be carefully contoured while the CO₂ laser is used to refine the surface and improve overall texture.

If you are looking for rhinophyma treatment in Houston, a comprehensive evaluation can determine whether procedural treatment is appropriate and which approach is best suited to your anatomy, skin, and degree of tissue overgrowth.

What Is Rhinophyma?

Rhinophyma is an advanced form of phymatous rosacea characterized by progressive thickening and enlargement of the skin of the nose. The sebaceous glands, connective tissue, and skin can become enlarged and fibrotic, resulting in a nose that appears increasingly irregular or bulbous.

Early changes may include:

  • Persistent redness of the nose
  • Visible small blood vessels
  • Enlarged pores
  • Increasing skin thickness
  • An oily or rough skin texture

As rhinophyma progresses, the nasal contours can become increasingly distorted, with nodular or lobulated areas of excess tissue.

Rhinophyma is much more common in men and typically develops later in adulthood, although it can occur in anyone.

What Causes Rhinophyma?

Rhinophyma is associated with rosacea, a chronic inflammatory skin condition that can cause facial redness, flushing, visible blood vessels, and inflammatory lesions.

The exact reason some patients with rosacea develop significant phymatous changes is not completely understood. Long-standing inflammation and enlargement of sebaceous glands and connective tissue appear to contribute to the development of rhinophyma.

Does alcohol cause rhinophyma?

No. Alcohol does not directly cause rhinophyma. However, alcohol can trigger facial flushing in some patients with rosacea and may make existing redness more noticeable.

What Does Rhinophyma Look Like?

Rhinophyma can range from relatively subtle thickening to significant enlargement and distortion of the nose.

Common features include:

  • A larger or more bulbous nose
  • Thickened nasal skin
  • Enlarged pores
  • Irregular or nodular tissue
  • Increased redness
  • An oily or uneven skin surface
  • Distortion of the nasal contours

In more advanced cases, the excess tissue can become substantial enough to interfere with hygiene or, in severe cases, nasal airflow.

Can Rhinophyma Be Treated Without Surgery?

Medications used to treat rosacea can help control inflammation and may reduce the risk of further progression. However, medications generally cannot remove established excess tissue once significant rhinophyma has developed.

When there is substantial thickening or distortion of the nose, procedural tissue reduction is typically necessary to restore the nasal contour.

Several techniques have been described, including surgical debulking, dermabrasion, electrosurgery, radiofrequency techniques, and laser resurfacing. There is no single treatment that is considered universally best for every patient, so treatment should be individualized according to the severity and anatomy of the rhinophyma.

What Is the Best Treatment for Rhinophyma?

For established moderate to advanced rhinophyma, the goal is to remove excess tissue while preserving the underlying nasal structures and creating a smooth, natural contour.

At Facial Plastic Surgery Associates, Dr. John Hamati and Dr. Sean Delaney commonly use a combination of radiofrequency debulking and sculpting with CO₂ laser resurfacing.

Rather than relying on the CO₂ laser alone, the combination approach allows the bulk of the excess tissue to be reduced and sculpted first, followed by laser treatment to refine the surface.

How Does Radiofrequency Debulking Treat Rhinophyma?

Radiofrequency energy can be used to carefully reduce and sculpt the excess tissue associated with rhinophyma.

The objective is not simply to make the nose smaller. The tissue is selectively reduced to recreate more natural nasal contours while preserving appropriate tissue thickness and support.

Radiofrequency sculpting can be particularly useful when there is substantial tissue overgrowth that needs to be debulked before the skin surface is refined.

How Does CO₂ Laser Treatment Help Rhinophyma?

CO₂ laser resurfacing can be used after tissue debulking to refine the nasal surface and improve irregularities in texture and contour.

The laser allows the surgeon to carefully resurface areas of uneven or thickened skin and create a smoother transition between treated and untreated areas.

CO₂ laser treatment has been used extensively for rhinophyma and can produce durable improvement when appropriately selected and performed.

Why Combine Radiofrequency and CO₂ Laser Treatment?

Rhinophyma is both a three-dimensional tissue problem and a surface-texture problem.

Radiofrequency debulking addresses the bulk of the excess tissue, while CO₂ laser resurfacing can refine the remaining contour and skin surface.

In other words:

Radiofrequency reduces and sculpts the excess tissue.
CO₂ laser treatment refines the contour and surface.

This combination allows treatment to be tailored to the individual patient’s anatomy rather than relying on a single technique for every case.

How Is Rhinophyma Treatment Performed?

During your consultation, Dr. Hamati or Dr. Delaney will evaluate the thickness, distribution, and severity of the rhinophyma as well as your overall nasal anatomy and skin characteristics.

Treatment is typically performed using local anesthesia, with additional anesthesia or sedation depending on the extent of treatment and the patient’s needs.

The excess tissue is first carefully reduced and sculpted using radiofrequency. Once the desired overall contour has been established, CO₂ laser resurfacing can be used to refine the surface.

The objective is a nose that looks smaller, smoother, and more naturally contoured—not simply a nose with tissue removed from it.

Is Rhinophyma Treatment Painful?

Local anesthesia is used to keep the treatment comfortable. Some patients may experience pressure or sensations during the procedure, but significant pain should be minimized. If they are healthy enough, many patients may prefer to undergo the procedure with IV sedation to keep them comfortable throughout.

After treatment, patients can expect some degree of redness, swelling, sensitivity, and drainage as the skin heals.

What Is Recovery Like After Rhinophyma Treatment?

Recovery depends on the severity of the rhinophyma and the extent of treatment.

Immediately following treatment, the nose will typically appear red and swollen. As the treated skin heals, patients may experience drainage, crusting, and peeling.

During the initial healing period, careful wound care is important.

Many patients begin to see significant improvement as the skin heals over the first several weeks. Redness can persist beyond the initial healing period and gradually fade over time.

Your specific recovery instructions will be provided based on the technique and extent of your treatment.

Will Rhinophyma Treatment Leave a Scar?

The goal of treatment is to restore a smooth, natural contour while minimizing visible scarring.

However, any procedure that removes or resurfaces tissue carries some risk of pigment changes, scarring, prolonged redness, or contour irregularity. The risk can vary depending on the extent of rhinophyma, skin type, treatment technique, and individual healing.

Because the nose has highly visible and anatomically complex contours, careful treatment planning is particularly important.

Are Rhinophyma Results Permanent?

Treatment can provide long-lasting improvement, but it does not eliminate the underlying tendency toward rosacea.

Because rosacea can continue to affect the skin, recurrence or additional thickening is possible over time. Maintaining appropriate rosacea treatment, protecting the skin from ultraviolet exposure, and working with a dermatologist when appropriate can help manage the underlying condition.

Can Rhinophyma Come Back?

Yes. Rhinophyma can recur or continue to progress because the underlying rosacea and inflammatory tendency remain.

That does not mean recurrence is inevitable. Appropriate management of rosacea and long-term skin care can help reduce the likelihood of significant progression.

Can Rhinophyma Affect Breathing?

Severe rhinophyma can sometimes interfere with the nasal openings or alter the external nasal anatomy enough to affect airflow.

For patients with significant functional symptoms, the treatment plan should address both the appearance and function of the nose.

Is Rhinophyma Cancer?

Rhinophyma itself is not considered a precancerous condition. However, skin cancers—including basal cell carcinoma—have been reported within areas affected by rhinophyma.

Any suspicious, ulcerated, bleeding, or otherwise concerning area should be evaluated appropriately and may require biopsy and pathology examination.

What Is the Difference Between Rosacea and Rhinophyma?

Rosacea is a chronic inflammatory skin condition that can cause facial flushing, redness, visible blood vessels, and inflammatory lesions.

Rhinophyma is a phymatous manifestation of rosacea in which the skin and underlying tissue of the nose become progressively thickened and enlarged.

Not everyone with rosacea develops rhinophyma.

When rhinophyma becomes established, treating the underlying rosacea alone generally cannot remove the excess tissue that has already developed.

Who Is a Good Candidate for Rhinophyma Treatment?

You may be a candidate for treatment if you have:

  • Progressive thickening or enlargement of the nose
  • A bulbous or irregular nasal appearance
  • Enlarged pores or thickened skin
  • Nodular or excess tissue
  • Difficulty maintaining hygiene because of excess tissue
  • Cosmetic concerns related to rhinophyma
  • Functional concerns related to nasal obstruction

During your consultation, your surgeon will determine whether procedural treatment is appropriate and discuss the options based on your individual anatomy.
